Requirements when applying change the name of a licensed premises.
When a licensee wishes to change the current approved trading name at a licensed venue an application must be lodged to request the approval of the licensing authority prior to the name being changed.
This application is not used for situations where the licensee wishes to change their company name (not the trading name). Where the entity conducting business under the licence will be changing please refer to the Transfer of Licence.
Applicants must lodge completed applications at the department. An application should be made using the forms provided below.
Department staff will be available between 8.30 am and 4.00 pm to assess your application to ensure that it meets the legislative requirements prior to lodgement.

application fee is not refundable, even if the application is
refused or withdrawn.
